Prepare evidence for your route
For program participation, a benefits letter or current portal record should identify the participant, issuing organization and program. For income, accepted examples include a tax return, annual income statement or three consecutive months of income records. Follow the official request for the required dates.
Make the records readable
Check that names, dates and relevant amounts are visible. If automatic verification fails, additional identity, address or household documentation may be requested. A proof-of-program document and a proof-of-identity document serve different purposes.
Use the official submission channel
Keep originals and submit the copies requested by the application system. Do not send identity documents, account PINs or Social Security details to Free Phone. If a request is unclear, ask the Lifeline Support Center or the participating company before submitting.
