Phone Availability

Required Documents — General Lifeline Guidance

  • Program route: a current benefit letter, statement or benefits-portal record identifying the participant, program and issuing organization.
  • Income route: an accepted tax return, annual income statement or three consecutive months of income records. Follow the documentation request for dates and household members.
  • Identity and address: provide accepted proof if the application cannot verify these automatically.
  • Household information: an additional worksheet may be needed when separate households share an address. Submit documents only through the official application process.

Application Information — General Lifeline Guidance

  • In most states, apply through the official Lifeline application system, by mail, or with help from a participating company. Oregon and Texas use state processes.
  • Provide additional documents if the eligibility review requests them.
  • After approval, choose a participating company and enroll. Program approval and starting a service plan are separate steps.

  • Federal Public Housing Assistance
  • Veterans Pension or Survivors Benefit

Qualify by Income

  • Federal Lifeline uses household income at or below 135% of the applicable poverty guideline. The threshold varies by household size and by Alaska, Hawaii or the rest of the US.
  • Choose your state in the eligibility directory for the 2026 table. State supplements can differ.
